What are hives?
The 2026 international guideline defines urticaria as a frequent, mast cell-driven disease. It involves the rapid appearance of wheals (raised, itchy welts), angioedema (deeper swelling), or both.E2Good1
Chronic urticaria is divided into spontaneous and inducible forms. It is disabling, harms quality of life, and affects performance at work and school. New treatments are available.E2Good1
What antihistamine labels say
The Zyrtec Hives label says it reduces hives and relieves itching. It also says it will not prevent hives or an allergic skin reaction from occurring.Official text3
The same label says avoiding the cause of hives is the only way to prevent them, and that hives can sometimes be serious.Official text3
It warns that some symptoms with hives may be signs of anaphylactic shock. It also warns that the product is never a substitute for a prescribed epinephrine injector.Official text3
The Benadryl label warns that marked drowsiness may occur.Official text4
What is mast cell activation syndrome?
Experts who proposed a diagnostic approach explain that MCAS can be diagnosed in severe cases, but overt MCAS is unusual. Many people referred for it are found to have other diseases, or allergic conditions that do not meet MCAS criteria.E2Good2
They add that symptoms are sometimes misread as MCAS while other important conditions go unexamined.E2Good2
The two main conditions that can underlie MCAS, they write, are severe allergy and systemic mastocytosis.E2Good2
US anaphylaxis guidance calls serum tryptase important for finding underlying mast cell disorders.E2Good5
What is not known yet
This guide is short because the verified library holds few sources on chronic hives and mast cell disorders yet. More are planned.
Questions you might bring to your clinician
- Are these hives acute or chronic, and spontaneous or triggered by something?
- Is there any sign that these episodes are anaphylaxis rather than hives alone?
- Would a tryptase level or a referral help here?
- If MCAS has been suggested, which other conditions have been checked?
