How often does it happen?
In a US multicenter study of 19,575 patients, 25 cases of awareness were identified (0.13%). That is about 1 to 2 per 1,000 patients at each site.E2Good1
There were 46 more possible cases and 1,183 cases of possible dreaming during anesthesia.E2Good1
The UK's Fifth National Audit Project found certain, probable and possible awareness in about 1 of every 19,600 anesthetics.E2Good2
Why does it happen?
Most cases reported to the UK audit were unintended awareness while a muscle relaxant was in effect.E2Good2
Muscle relaxants such as rocuronium have no effect on consciousness or pain. That is why the label says they must be given with adequate anesthesia or sedation.Official text3
On waking, some patients experienced leftover muscle weakness as awareness, often because full muscle strength had not returned.E2Good2
Questions you might bring to your anesthesia team
- Will a muscle relaxant be used, and how will its reversal be confirmed?
- How will the depth of my anesthesia be monitored?
- I had awareness before: how will that change the plan?
